Pursuit of Longevity Medical Corporation · version 2

Terms of use

Effective 2026-11-03. Working version prepared by the clinic; under review by counsel.

Who we are

The POL Portal ("the portal") is operated by Pursuit of Longevity Medical Corporation ("the clinic", "we"), a California professional medical corporation. The clinic provides physician-led telehealth care to residents of California. By creating an account or using the portal you agree to these terms.

The portal is not for emergencies

The portal does not provide emergency or urgent care and is not monitored around the clock. If you have a medical emergency, call 911 or go to the nearest emergency room. Messages sent through the portal are answered during the clinic's working hours.

Who may use the portal

You must be 18 or older and physically located in California at the time of any telehealth visit. You agree to give accurate, current and complete information in your account, your intake questionnaire and your messages, and to keep it up to date. The clinic relies on what you tell it.

Your account

Sign-in is by a one-time code sent to your mobile number or by a sign-in link sent to your email address; there is no password. You are responsible for keeping your phone and email account secure and for everything done through your account. Tell the clinic at once if you believe someone else has used it.

Telehealth and the physician relationship

Using the portal does not by itself create a physician–patient relationship. That relationship begins when a clinic physician accepts you as a patient after reviewing your intake, your consents and, where required, your laboratory results. The physician decides what care, if any, is appropriate; the portal records and carries out that decision. Nothing the portal shows you before a physician has signed a decision is medical advice.

Laboratory testing

Baseline and follow-up testing is performed through a third-party laboratory partner using an at-home collection kit shipped to you. Results are released to you only after a clinic physician has reviewed them. Critical results are escalated to the physician directly.

Medications, compounded products and supplements

Some therapies the clinic may prescribe are compounded medications. Compounded medications are not approved by the FDA; they are prepared for an individual patient by a licensed compounding pharmacy on a physician's prescription. The clinic may also recommend dietary supplements; the clinic has a financial interest in its supplement dispensary and discloses this to you in writing before any purchase.

Fees and membership

Fees, what they cover, and renewal and cancellation terms are set out in the Membership terms you sign separately. Laboratory testing and medications may be charged separately as disclosed at the time.

Text messages

If you opt in, the clinic sends account notifications by SMS: sign-in codes, reminders that something in the portal needs your attention, and kit delivery updates. Never clinical content, never marketing. Message frequency varies; message and data rates may apply. Reply STOP to opt out, HELP for help. The full Text message terms are published on the portal.

Acceptable use

You agree not to use the portal to impersonate anyone, to obtain care for another person, to upload content you do not have the right to share, to interfere with the portal's operation, or to attempt to access another person's information.

Content and intellectual property

The portal, its text, design and software are the property of the clinic or its licensors. You may use them only to receive care from the clinic. Your own health information remains yours; your rights in it are described in the Privacy notice.

No warranty; limitation of liability

The portal is provided as is. To the fullest extent permitted by California law, the clinic is not liable for indirect, incidental or consequential damages arising from use of the portal itself (as distinct from the medical care you receive, which is governed by the applicable standard of care and by law). Nothing in these terms limits liability that cannot be limited by law.

Ending your use

You may close your account at any time by asking the clinic. The clinic may suspend or close an account for misuse. Your medical record is retained as required by California law regardless.

Changes

The clinic may update these terms. The version in effect is shown on this page with its effective date; material changes are notified in the portal before they take effect.

Governing law

These terms are governed by the laws of the State of California. Any dispute is brought in the state or federal courts located in California.

Contact

Write to the clinic through the portal's message center, or by mail at the clinic address shown on your membership documents.
