Privacy notice
Effective 2026-11-03. Working version prepared by the clinic; under review by counsel.
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Who this notice covers
Pursuit of Longevity Medical Corporation ("the clinic", "we") is a health care provider covered by the federal Health Insurance Portability and Accountability Act (HIPAA) and by the California Confidentiality of Medical Information Act (CMIA). This notice applies to the clinic, its physicians and staff, and to the POL Portal through which you receive care.
What we collect
Identity and contact details you give us (name, date of birth, address, email, mobile number); a photo of a government ID when identity verification is required before a prescription is transmitted; your intake questionnaire and messages; laboratory results; the decisions, plans and prescriptions your physician records; billing information; and technical records of your use of the portal (sign-in events, device and browser type, the time pages and documents are opened). Payment card details are handled by our payment processor and are not stored by the clinic.
How we use and share your information
For treatment: to evaluate you, to decide on and deliver care, and to coordinate with the laboratory, the pharmacy and other providers involved in your care. For payment: to bill you and to process membership fees and other charges. For health care operations: quality review, training, audit, security and improvement of the portal.
With your written authorization only: sharing laboratory results with the clinic's laboratory partner (so results reach your physician), transmitting prescriptions to the dispensing pharmacy, and any optional sharing you choose, such as with a third-party health-tracking service. Each authorization is a separate document you sign in the portal and may revoke at any time; revocation does not undo sharing that already happened.
As required or permitted by law: public health reporting, reporting of abuse or neglect, health oversight, judicial and administrative proceedings, law enforcement where required, to avert a serious threat to health or safety, and as otherwise required by law.
We do not sell your information. We do not use it for marketing. Mobile numbers collected for text notifications are not shared with third parties or affiliates for marketing or promotional purposes.
Decision-support software and artificial intelligence
The clinic uses software, including generative artificial intelligence, to help organize intake information and laboratory results and to draft clinical content. A clinic physician reviews anything produced this way before it reaches you or your record. California law (AB 3030) requires us to tell you this, and we do so in a separate disclosure you sign.
Service providers
The portal runs on cloud infrastructure in the United States. The clinic uses service providers for hosting, email, text messaging, identity verification, laboratory logistics, pharmacy dispensing and payment processing. Each is bound by contract (including a business associate agreement where HIPAA requires one) to protect your information and to use it only to provide the service.
Your rights
You have the right to: see and get a copy of your record, including an electronic copy (the portal provides a record export under Account › Documents); ask us to correct information you believe is wrong; receive an accounting of certain disclosures; ask for restrictions on how we use or share your information (we are not always required to agree, but we must honor a request to restrict disclosure to a health plan for a service you paid for in full); ask to be contacted in a particular way or at a particular place; receive a paper copy of this notice; and revoke an authorization in writing at any time.
Our duties
We are required by law to keep your information private, to give you this notice, to follow the notice currently in effect, and to tell you if a breach of your unsecured information occurs.
Security
The portal uses encryption in transit and at rest, signed-in access only, role-based access for clinic staff, and an access log that records who opened which part of your record and when. Document signatures are recorded with the exact document version and a cryptographic hash.
Retention
Medical records are retained for at least the period California law requires for adult patients, and longer where another law requires it. Account and billing records are kept as needed for the purposes above.
Children
The clinic treats adults only and the portal is not intended for anyone under 18.
Changes to this notice
We may change this notice; the current version with its effective date is always published on the portal and applies to all information we hold.
Complaints and questions
If you believe your privacy rights have been violated you may complain to the clinic through the portal's message center or by mail at the clinic address shown on your membership documents, and to the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
